The Perioperative Surgical Home is an innovative care delivery model focused on the coordination of care for the surgical patient population, from decision for surgery through the entire perioperative/ surgical continuum. The Clinical Nurse Coordinator (CNC) promotes the delivery of evidence:based interventions for a specific surgical patient population, by identifying and clarifying patient needs, coordinating healthcare services, conducting health education, promoting patient advocacy and evaluating patient outcomes Responsibilities: Coordinates a multidisciplinary, collaborative healthcare team in the management of specific surgical patient populations through the perioperative continuum via the Perioperative Surgical Home. Collaborates with the appropriate disciplinary teams to ensure scheduled tests, diagnostic procedures, pre / post op care and appointments are coordinated. Demonstrates commitment to patient safety by identifying and addressing safety issues and changing practices to enhance safety. Acts as a patient advocate in developing and facilitating a plan that respects the patients culture and values, individual needs, health goals and treatment preferences. Provides education to the patients and families regarding the perioperative continuum of care. Serves as a liaison, consultant and resource for patients, physicians, and other health care professionals involved in the care of a specific patient population. Actively participates in quality improvement initiatives, utilizing information from relevant sources, such as databases and benchmarks. Maintains accurate and current knowledge base of trends, issues, research findings and local, state and national health related resources and services for the specific patient population services are being provided. Demonstrates leadership through participating in department and hospital committees, councils and taskforces. Demonstrates knowledge of internal and external resources and coordinates necessary resources to achieve optimal outcomes Requirements: High School Diploma or GED required Bachelors Degree in Nursing required Masters Degree in Nursing, or related field within 3 to 5 years required Minimum of three (3) years of perioperative/procedural experience required Certification Preferred: CNOR, CPAN, CCRN, CAPA If selected to move forward in the hiring process, you will receive an email invitation to schedule your interview for this position.
